Does anyone know how to unlock the break through mortal limits achievement?
By breaking through your mortal limits. I think there are multiple ways. I don't play Mortal Limits much, but I remember you can do it in the Dragon Trial. I think it was by becoming the Wind Spirit's Vessel.
